Riley County is seeking applications for the position of Community Care Team Coordinator. The Community Care Team is a pilot project intended to improve access to, and coordination of, healthcare services in the Flint Hills area. The team's success has been measured in a marked cost avoidance for several healthcare entities and improved outcomes for its patients. The Community Care Team Coordinator will be tasked with managing the increasing scope of the Community Care Team as well as working with area partners on a sustainability and scale-ability plan. This is a part-time fixed term, grant funded position. Funding for year two is contingent on meeting grant requirements and authorization from the funding agency. Funding is provided through the Memorial Hospital Association. The Community Care Team Coordinator will manage the efforts of the Flint Hills Community Care Team (CCT), which operates under the Flint Hills Wellness Coalition, in order to improve access to, and coordination of, healthcare services in Riley County in an effort to improve patient outcomes and to eliminate the costly and inefficient overutilization of emergency medical services. This position is considered a leadership position requiring strong communication skills to collaborate with community members, healthcare providers, and the Flint Hills Community Care Team members to address healthcare efficiency and accessibility issues and in Riley County.
